Support towards Independent Living Flatlets
tracscare view supported living as an extension of its residential care service and its flatlets/semi-independent care service. In this sense it is the pen-ultimate step on the pathway of care. Supported living allows clients to move back into the community in a true sense with their own accommodation but with support tailored to their needs. This may be appropriate for all client groups dependent upon individual need, but is generally most associated with Acquired Brain Injury or Mental Health client groups. Prospective clients will be assessed and a proposal for appropriate hours of support will be agreed. The nature of the support will also be agreed within the supported living care plan, but this may cover general support, emotional support, problem solving, budget planning, meal planning, shopping, medication management, mental health relapse indicator management, etc.
Occasionally, tracscare may also provide Supported Living/Domiciliary care for clients with less independent skills but with the specific need to live alone with staff support for whatever reason. In such cases a comprehensive assessment of needs will determine the level of support provided.
