Clinical Support

At tracscare we support our care teams with a variety of Clinical Input, subject to clients' individual needs. Support is often provided from the clients' local team, but where this is not provided or does not fully meet the clients full needs we will complement local services with our own team. The input of external consultancy is agreed at the assessment/admission stage or at client IPP reviews with multidisciplinary input and approval. The clinical support available to tracscare clients includes the following:

Clinical Support Nurses

tracscare employs Clinical Support Nurses both from Learning Disabilities and Mental Health trained backgrounds to support the care teams. Their primary role is to advise on clinical assessments, care Planning and Risk Management, but they also provide valuable clinical advice, guidance, training and mentoring to the care teams. It is unusual to find Nursing functions within a social care setting but at tracscare we wanted to be different and add value for our purchasers and clients.

Our Clinical Support Nurses are also used to liaising with external clinical consultants, coordinating visiting clinics, accessing assessments etc.  tracscare also has trained nurses at Director and Senior Manager level.

Behavioural Management

tracscare has employed Behaviour Advisors for several years.  Studio III is a training system which incorporates a Reactive Management philosophy.  It is based on Positive principles and approaches to challenging needs,  The training focus is on low arousal, non-confrontational interventions and has proved excellent in reducing challenging incidents; thus allowing successful placements for clients who may have a history of placement breakdown.  Our employed Behaviour advisors have a two fold role; they provide comprehensive training for all staff which is followed up with an annual refresher.  They also provide both an advisory or emergency intervention role which allows them to work hands on alongside the care team, where challenges are being experienced.  Our Behaviour advisors work with the care team in a consultative way to establish Reactive Management plans which focus on the identification of triggers and de-escalating strategies.

To complement this core clinical support tracscare also retains or accesses as required, Clinical Consultancy according to clients individual basis.

Behaviour Therapy

tracscare have access to the services of a Behaviour Therapist, from the American Positive Programming School, whose positive philosophy matches our own.  He assists with Behavioural Assessment and functional analysis.  He also provides advice on strategies and interventions with the aim of increasing positive behaviours and finding alternative behaviours to fulfill the functions of challenging behaviours currently exhibited by the client.

Psychology

tracscare has retained services from a number of clinical psychologists with varied clinical areas of expertise (Acquired Brain Injury/Neuro-Psychology, Mental Health, Challenging Needs and ASD) whom we access for psychological assessment and advice re support strategies, care plans etc.  We also use Clinical Psychologists to undertake individual pieces of work direct with clients as required, such as counseling, cognitive behaviour therapy etc.

Psychiatry

tracscare has retained the services of a number of Psychiatric Consultants who can be accessed to provide medication monitoring through consultation with the clients GP.

Art Therapy

tracscare has retained the service of an experienced art therapist, who works with Challenging Needs, Challenging Need, Personality Disorder, ASD client groups.  He usually undertakes direct work with the client, on a time limited, regular basis.

Speech Therapy

Assessments for speech therapy can be arranged when required. tracscare has access to a Neuro-Speech Therapist for our Acquired Brain Injury client group.

Other Clinical Services

The services of Physiotherapists and Occupational Therapists are normally accessed from local services via the GP, but can be accessed on a consultancy basis if required.

Clinical Assessments and Outcomes

In addition to the initial needs assessment tracscare undertakes a detailed clinical assessment, normally within three months of admission, for each client placed in our service. This assessment monitors the effect of a specific diagnosis on a clients functioning within a residential care setting. This process, unique to tracscare, has specific assessments for Autistic Spectrum Disorder, Challenging Needs, Mental Health and Acquired Brain Injury. All of our clinical tools have been devised with the input of Psychologists, Neuro-Psychologists, Behaviour Therapy or the Autistic Accreditation service.  tracscare also utilizes an Independent Lifeskills Assessment tool.  These assessments help with care planning with the aim of reducing needs and increasing client independence and community presence.  This approach allows tracscare to adopt a Rehabilitative focus and allows for the measurement of clinical outcomes in a residential setting.
