tracscare - Our Care Policy

It is widely accepted that a person with a disability should be able to live a normal life. At tracscare we aim to provide a normal life style for our clients, where each person is able to enjoy choice, freedom, independence and an environment conducive to continued development towards self reliance.

We feel that a person with a disability needs a more structured life to be able to develop towards their maximum ability. To this aim, a person centred care plan is written for each individual. The client, together with his/her relatives and others, are encouraged to take part in care plan meetings and reviews.

tracscare provides a varied clinical service to cater for all client needs. We employ qualified Clinical Support Nurses (learning disability or mental health trained), who co-ordinate clinical input including; advice on care plans; setting up and overseeing risk assessments; 1:1 support for clients, offering a consensual approach towards the setting up and review of support and reactive management plans; producing clinical booklets and providing training to assist in staff development, on subjects such as challenging behaviour, epilepsy, learning disability, brain injury. The Clinical Support Nurses also share up to date knowledge and research with staff, company wide.

Specialist Input

Where required, tracscare arranges input for Speech and Language Therapy, Behavioural Therapy, Art Therapy, Clinical Psychology and Psychiatry. In addition, reflexology and other alternative therapies are available as and when necessary.

In 2005, tracscare were pleased to have retained the services of Professor Rodger Wood and his team, to provide neuropsychology services to our acquired brain injured clients. In particular, Professor Wood and his team developed an acquired brain injury assessment tool to assist tracscare to identify and address cognitive deficits and the effects of neuro behavioural disorder



tracscare - A Summary of Our Care Aims

  • A skilled diagnosis of the needs of those entering the home, a personal programme of care and training to meet those needs, and regular reviews.
  • A selection/admissions procedure which allows for a trial period and a balanced assessment of difficulties likely to be encountered by the new resident and by the existing residents or staff, before admission is finalised.
  • Provision of the supportive relationship offered by a named keyworker of client choice (where feasible)
  • Provision of therapuetic activity that may or may not be socially useful but provides stimulation and interest, and is not viewed as work.
  • Provision of training that is monitored whether through leisure activities, work, therapy or adult education.
  • Links fostered with organisations and individuals in the local community which will encourage residents to move out of the residential setting for their social contacts.
  • The availability of transport.
  • The fostering of links with families and friends, including the encouragement of family members and friends to visit.
  • Provision for residents to take holidays.
  • Provision of the possibility of single accommodation to those needing or desiring it.
  • Regular meetings, in whatever form, where residents and staff can express their views on matters relating to their daily life.
  • Provision, by a known procedure, for residents' complaints to be considered and dealt with.
  • Opportunity for religious practice, if desired, without coercion.
  • The maintenance of a constant look-out for opportunities whether of work or accommodation, which might provide the means for some residents to attain greater independence.
  • Encouragement to use normal communal facilities such as Doctor, Dentist, public transport, pubs, shops, cinemas.
  • Every encouragement for each person to have his/her own furniture and belongings, and to choose type of decor and colour schemes.
  • Regular liaison with all sources of assistance in providing the necessary diagnosis, education and training.
