Finding a suitable home for a service-user with Dual Diagnosis, who requires a high level of support needs, may not be a quick or easy process for Social Workers. However, there are a few lucky clients, who have settled into their new homes in Shropshire and The new homes are ‘Glanmore’, situated in The two homes opened in 2006, operated by tracscare, are registered by CSCI for adults with a Dual Diagnosis. Both homes are recently refurbished properties with a quality finish boasting new kitchens, leather sofas, and modern décor. Glanmore House has seven en-suite bedrooms, and in addition to the usual homely communal space, has a very large conservatory. Glanmore has disabled access to ground and first floors. At tracscare, we believe that a person with a disability should be able to live a normal life to enjoy choice, freedom, independence and an environment conducive to continued development towards self-reliance. We feel that a person with a disability needs a more structured life to be able to develop towards their maximum ability. To this aim, a person centred care plan is written for each individual. Clients also have access to a variety of clinical services according to their needs. tracscare employs qualified Clinical Support Nurses (learning disability or mental health trained), who co-ordinate clinical input.
