tracscare - The Willows

Location

The Willows is situated in a small village, just outside Neath. The Home is only a short 5 minute walk from shops, pubs and other local amenities and a 10 minute bus ride to the towns of Neath and Pontardawe.

Accommodation

On the ground floor, the Willows has four bedrooms, a communal lounge, kitchen and separate smoking lounge. On the second floor there are four bedsits, each with their own kitchenette suitable for fully mobile people, a lounge and a smoke room. This combination of accommodation allows for the progression of clients into the upstairs units. The Home has a large landscaped garden.

Client Group

The Willows offers care for clients with an acquired brain injury and/or borderline learning difficulties/mental health. Clients in the Willows are at a stage of their rehabilitation where they are able to focus on their independent life skills.

Activities

Clients at the Willows have available and are encouraged to access, a wide range of community facilities including the Leisure Centres, public library, shops, supermarkets, colleges, restaurants, pubs, cinemas etc., are of which are within a 10 minute bus ride away. Supported work placements are encouraged.

Staff

The Willows usually has two, sometimes three staff on duty, Monday to Friday and two at weekends, who are available to support clients at any time. Client's individual support plans will determine the level of support available. The staff are trained to build client confidence and encourage independence. There is wake night support for downstairs which is available to support bed-sit clients as required.

Care

The Willows provides safe, but discrete, individualised support towards an independent lifestyle. This support is based on tracscare independent lifeskills assessment process. The home's care policy is outlined fully within tracscare Care Policy.

Clinical Input

Each client at The Willows has an individual care plan, which is regularly monitored and updated by tracscare Clinical Support Nurse. There is access to a Psychologist on a monthly basis. The Health Centre is only 10 minutes away in Neath. The Willows is registered for 8 beds (4 bed-sit units and 4 more dependent beds) for adults with Learning Difficulties/Mental Disorders by the Care Standards Inspectorate Wales.
