![]() tracscare - James StreetLocationJames Steet, is comprised of two 3-bedded small homes, situated in Pontardawe, within easy reach of both Neath and Swansea and close access to the M4 motorway. The Home is close to village shops, bus stops and local amenities. AccommodationJames Street consists of two, traditional, turn of the century, terraced houses which is registered as one unit. James Street has three floors and provides accommodation for fully mobile clients. The Home offers a homely atmosphere and is focussed on supporting clients towards independent life skills at Number 9 and providing more intensive staff support at Number 11. James Street (9 and 11) each have three single occupancy bedrooms, each has a communal bathroom, a communal lounge and their own kitchens. Both homes share a large garden with a fishpond. Each house has access to a vehicle, but use of public transport is encouraged to promote independent life skills. Client GroupJames Street provides support towards independent living for six clients. Number 9 caters for the needs of brain injury clients and Number 11 caters for the needs of dual diagnosis (learning disability and mental health) clients. ActivitiesJames Street is situated in the centre of Pontardawe close to supermarkets, shops, restaurants, pubs etc. There is a local centre within walking distance, which runs evening classes in maths, english, computers etc. The leisure centre is a five minute walk away as is the doctors and the library. Clients participate in team social activities such as badminton, snooker etc on a regular basis. StaffOn average James Street has 1 or 2 staff on duty at Number 9 and 2 or 3 on duty at Number 11 supported by a wake night staff and a 'sleepnight' staff member on duty overnight. All staff receive extensive internal training supplemented by relevant external courses. Staff receive in-depth training and on-going support on low arousal techniques, which are the core of tracscare care philosophy. Training is outlined in more detail on the Staff Page. Clients also have access to clinical training booklets, enabling them to have insight into how the staff are providing therapuetic support to clients to maximise individual potential. ClinicalA Clinical Support Nurse is employed full time by the company and works closely alongside the house manager and keyworker to ensure that the clients support needs are appropriately assessed and addressed with Care Plans. There is also consultancy support available for specific client groups. Clients may join tracscare Brain Injured Group, which promotes compensatory strategies to address residual cognitive deficits following acquired brain injuries. CareJames Street philosophy is 'Independent Lfe Skills through training and caring support'. Each client's individual rehabilitation needs are identified within tracscare Independent Life Skills Assessment Process, culminating in an individualised Support Plan. No two support plans are the same and each support plan is fluid, constantly being adapted to reflect client progress so each new client is welcomed by the team as unique. The home's care policy is outlined fully within tracscare Care Policy. James Street has achieved its mission statement with clients moving on and living independently. James Street is registered for 6 adults with learning difficulties/ brain injury/ mental health needs by the Care Standards Inspectorate Wales, who can be contacted on CSIW
